Reporting other types of incidents

 

If you believe you are the victim of identity theft, please contact local law enforcement as well as your insurance company’s Member Services; the number can be found on your insurance card. You may also wish to review information on the Federal Trade Commission website (www.ftc.gov), as that agency is involved with consumer protection.

 

Note that a complaint about a doctor, hospital, or service at a lab or pharmacy is not necessarily fraud. If you wish to make a complaint about the quality of your doctor’s service, have a problem with your bill, or have found something incorrect on your explanation of benefits (EOB), please contact your insurance company’s Member Services; the number can be found on your insurance card.
